Please I am still a bit confused in this church registration. I want to ask, the 2 persons(husband and wife), should I add them as chairman and secretary?

Please CAC just approved one of my company registrations where there are only 2 shareholders, but they duplicated a shareholder twice and put 0 shares alloted under one of the duplications ??‍♀️. Please help.
